Can I use both?
Yes. Some people keep a short TODO.md for the current branch and use Taskhold for the backlog across repos. Tell the agent in CLAUDE.md or AGENTS.md which one is the source of truth.
Does Taskhold read my TODO.md automatically?
No. Taskhold never reads your repository. An agent can copy items over with taskhold_create when you ask it to.
